What is Compass Care?
Compass Care is GLRC’s care coordination and health support program. Compass Care uses a “whole person” approach, recognizing that a client’s mental health and physical health can also be affected by many outside stressors. Clients may struggle with transportation barriers, housing instability, financial stressors, insurance issues, food insecurity, or difficulty accessing healthcare and community resources.
Compass Care team members, also known as CHW’s (Community Healthcare Workers) work closely with counselors, medical providers, peer supports, and other care team professionals to help coordinate care and improve communication between services and providers.
What does Compass Care provide?
Here are some of the areas that the Compass Care program may assist with:
• Care coordination between providers and services
• Connecting to medical or specialty healthcare providers
• Scheduling or coordinating appointments
• Insurance support and MDHHS application assistance
• Transportation resources
• Food or financial assistance resources
• Health education and wellness support
• Accessing community resources and support systems
• Coordination following hospitalizations or emergency room visits
• Medication follow-up and healthcare navigation
• Support with communication between providers, agencies, and treatment teams
